Please submit the CDI Workshop Request form below. After receiving your completed form, Student Affairs will contact you within 2-3 business days to set up a scheduled time to further discuss your workshop goals.

Please note: we require a minimum of 2 weeks’ notice to allow for adequate planning. Evening and weekend hours are limited to staff availability.
